Tl;dr -- All in all a decent book with a very specific audience. But it may be colored by your political lens. I am not a fan the way "monopoly" was borrowed and redefined: I don't much care for people redefining words in a way that dilutes the gravity of the original meaning. But the concepts he talks about here, of you can reframe them, can be useful and are insightful.

This is a pretty provocative book about startups and innovation, though some of its terminology (especially "monopoly") needs to be interpreted carefully.

Thiel is not really defending the conventional, antitrust-style idea of a monopoly (though I don't think he has a problem with it). He argues that the best companies create something so different from others that no competitor offers a close substitute, giving them room to earn durable profits and invest for the long term.
His argument is that competing head-to-head in crowded markets often destroys profits and pushes companies toward incremental improvements. Instead, a startup should try to create and dominate a small market (a micro-niche) then expand outward. To keep that advantage, it needs something difficult to copy: proprietary technology, network effects, scale advantages, a powerful brand, whatever. He considers this a "creative monopoly".

As is normal of Thiel, it is intentionally contrarian and sometimes overstates the case, but it is useful because it forces you to ask whether a business is truly creating new value or merely entering an already crowded category with a slightly different version of the same thing.
