Of note, it doesn't say in the offer page, but you only get YouTube Premium Lite included when the free trial ends. Also it did cancel and refund my existing annual Google one subscription automatically.

After selecting the perk on Verizon, did you go to Google one app? My Google one only shows free 3months. Trying to figure out how to get the offer
